Likeness Release for Adults and Minors
For any CAES materials that contain an image or video featuring a person, it is necessary to get permission to use that photograph of them. In the case of minors, this permission should be from the parent or guardian in writing.
If an OMC staff member is taking photos at an event/on location, he or she will be responsible for obtaining a completed consent and release form or verifying that a photo release has been signed and is available upon request. He/she will also keep any of these completed forms on file.
If a client of OMC is providing photos for publication purposes, the client is responsible for obtaining a completed consent and release form or verifying that a photo release has been signed and is available upon request. The client should keep any of these completed forms on file.
The upper right-hand corner of the consent form has empty space so a likeness photo can be attached.
